Annual Benefits Enrollment Period
The annual end-of-year benefits enrollment period
will begin immediately and end Friday, December 11, 2009. All changes made during this period will
be effective January 1, 2010. Please
review the information in this packet carefully.
See the section titled “How Do I Enroll or Make
Changes?” for instructions on enrolling in or making changes to your Health,
Dental, Vision and Flexible Spending Accounts during this enrollment period.
Medical, Dental, Vision Open Enrollment?
The University of Arkansas
is offering an open enrollment at this time for our medical,
dental and vision plans. All changes will
become effective January 1, 2010.
During Open Enrollment, benefits-eligible employees can: · Enroll for the first time in health, dental and vision coverage
· Add eligible but not currently covered dependents to your health, dental and vision plan
· Cancel your health, dental and vision plans or delete dependents from your health, dental and vision plans
· Change your health care election from the Classic Plan to the Point of Service Plans or vice versa
· Change your vision insurance to the new Enhanced Vision Plan (see Vision Coverage information below)
Remember, the University of Arkansas does not have Open Enrollment every year and dates for future Open Enrollment periods are unknown.
See the section titled “How Do I Enroll or Make Changes” for instruction on enrolling or making changes to your health, dental and vision plans. If you are currently enrolled in the health, dental or vision coverage plans and do not want to make any changes, you do nothing; your current coverage will remain the same. The following changes are planned for the
University of Arkansas

Smoking
Cessation Program – this is a new
program called “Kick the Nic Program.”
This program provides for a PCP office visit and prescription drug
coverage for Chantix at $0 co-pay.
Chantix is a prescription medicine to help reduce nicotine dependence. Contact QualChoice at 1-888-795-6810
for more information. Effective
January 1, 2010.

Diabetes Management Program –
members who have been diagnosed with diabetes and participate in this program
may receive their diabetic medicine and supplies at $0.00 copay. Contact QualChoice at 1-888-6810
for more information. Effective
October 1, 2009.

Changes
to Mental Health Benefit – the
UA’s medical plan has incorporated changes to comply with the Mental Health
Parity Act that will be effective January 1, 2010. These changes include the elimination
of visit limits. The first
mental health visit will be covered under the $40 office visit copay and will
not require prior approval.
Any additional visits will be covered only if there is a treatment plan
submitted by the mental health provider to QualChoice and approved by
QualChoice. For additional

Prescription Drugs – The
University changed pharmacy benefit managers (PBM) from Caremark to MedImpact
effective October 1, 2009. While the
UA’s PBM has changed, the co-pays and tier placement of prescription drugs
remains the same. The pharmacy plan
is still managed by the University of Arkansas Pharmacy Advisory Committee which
is comprised of pharmacy and medical professionals. This committee meets quarterly
to evaluate the pharmacy program and to make necessary changes based on the
latest evidence-based information available.
Any changes to the formulary will be communicated to you at least 30 days
prior to the effective date of the change.

Medical
Plan Premiums – Your medical plan
premiums will not be increasing as of January 1, 2010. The Chancellor’s Office recently sent
an e-mail confirming that the University will continue paying the employee’s
share of the July 1, 2009, premium increase through June 30, 2010.

Changes to
the Dental Plan
There will be no coverage changes or premium
increases to the UA dental plan effective January 1, 2010. See the Dental Plan Comparison for additional coverage information and premiums. Changes to the Vision Insurance Plan
Starting January 1, 2010, the University of Arkansas will offer two Superior
Vision Insurance plans, the Basic Plan and the Enhanced Plan.
The Basic Plan is our current
plan with no changes in coverage or premiums.
The Enhanced Plan offers some expanded benefits such as frames every
calendar year, increased spending allowances for contacts and frames, and
coverage for progressive lenses, scratch coating and UV coating.
The expanded benefits under the
Enhanced plan will require a higher month premium.
Current Superior Vision participants who want to remain on the Basic Plan will do nothing. Your current coverage will remain as it is. Current Superior Vision Participants who want to transfer to the Enhanced Plan must complete on-line enrollment or complete the necessary enrollment form to change their coverage. Employees enrolling for the first time must choose either in the Basic Plan or the Enhanced Plan. New enrollees who do not select a plan will be enrolled in the Basic Plan. See the “How Do I Enroll or Make Changes” section for instructions for enrolling or making changes.

Flexible Spending Account Enrollment
The annual open enrollment for FSAs (Health Care Reimbursement and Dependent Care accounts) will begin immediately and go through December 11, 2009. December 31 of each year. You must re-enroll by December 11, 2009 if you want to participate in 2010.
Consider enrolling in a
Health Care Reimbursement FSA if you have out-of-pocket medical expenses
like eye glasses, braces, prescription co-pays, insurance
co-pays/deductible/coinsurance, or over-the-counter drugs. The maximum annual contribution
to the health care FSA is $5,000; the minimum annual contribution is $120.
Consider a
Dependent Care Reimbursement FSA
if you have qualifying daycare expenses for dependent child under age 13 or an
elderly person or persons with a disability as long as they are claimed as
dependents for federal tax purposes.
The maximum annual contribution to the dependent care FSA is the lesser
of $5,000, your salary, your spouse’s salary or your eligible expenses; the
minimum annual contribution is $120.
· Enroll in Flexible Spending Accounts (Health Care Reimbursement and Dependent Care Reimbursement).
Annual enrollment required. ·
Enroll in health, dental or vision coverage.
·
Add or delete dependents from your health, dental, or
vision plans.
·
Change your health care elections from Classic to
Point of Service or Point of Service to Classic.
·
Change the status of health, dental, and vision
premiums from after-tax to pre-tax or pre-tax to after-tax.
All of the enrollments, cancellations, deletions and changes listed above can be made on-line through webBASIS on the University of Arkansas website. It’s quick and easy. Follow these steps:
Step1:
Log on to webBASIS at http://www.uark.edu/home/ and click on
webBASIS. Step 2: Log in using your UARK ID and UARK Password. If you do not know your UARK ID and Password, click on Accessing webBASIS. Follow the instructions there to find your UARK ID and password.
Step 3:
Click on Annual Benefits Enrollment.
Step 4:
START ~ Online Enrollment: In the Flexible Spending Account
section, complete your enrollment with your selected annual enrollment amount. Remember, your annual election must
be evenly divisible by your 9 or 12 month appointment.
You must enter $0.00 in these fields if you do not wish to participate. Step 5: Complete other Benefits enrollments or elections: Make any eligible changes to your health, dental and vision plans (see above) or do nothing with these sections. They will default to your current enrollment. Step 6: Click on VALIDATE: Any entry errors you have made will appear in red above the section with the error. Correct the error and click on VALIDATE again.
Step 7:
Click on SAVE to save your changes.
If you don’t click on SAVE before exiting
“My Annual Enrollment” and changes you made will be lost.
You can also get copies of the forms you need from this website or pick them up from Human Resources at 221 ADMN. Be sure to make a copy of any enrollment forms before sending them to Human Resources. Do not complete a paper enrollment form if you enroll on-line and do not enroll on-line if you complete a paper form.
