Critical Illness Insurance
Critical illness insurance pays a lump-sum benefit payment for these covered medical conditions: cancer, heart attack, stroke, kidney failure, major organ transplant, Alzheimer's Disease or coronary bypass graft.
The payment can be used for co-payments and deductibles, travel to treatment centers, child care, or other expenses not covered by insurance, at the discretion of the employee.
The cost for critical illness insurance depends on the selected coverage; see the Critical Illness Enrollment Kit for a summary of benefits, premiums and an enrollment application. New employees have 31 days from their benefits-eligible appointment dates to enroll in critical illness insurance. A separate application is required to enroll a spouse or dependent children.
