Employee, supervisor and reviewing official sign the last page of the approved evaluation form.


 

Employee receives copy of completed evaluation form (all pages).


 

Retain completed performance evaluation form in departmental personnel file.


 

Submit the top and last page (photocopy or original) of the approved form to Human Resources, 222 Administration Bldg, by the March 15 deadline.


 

Prepare Standards of Performance for the next evaluation period.
